ASSUMPTION OF RISKS
I/We understand that participation in the West Fifth Bible Chapel Chess Club involves certain risks. Although activities are primarily mental and social, some risks may include slips, trips, minor physical contact, and use of general building facilities. I/We freely and voluntarily assume all such risks and any resulting personal injury, illness, property damage, or loss arising during or resulting from participation.
DETAILS OF ACTIVITIES
The Chess Club will include instructional sessions, tournaments, and social activities held primarily at West Fifth Bible Chapel (440 West 5th Street, Hamilton, Ontario). Events may occasionally include off-site chess tournaments or social gatherings, for which participants will receive separate notice.
ASSUMPTION OF RESPONSIBILITY
I/We acknowledge that it is our responsibility to ensure that the participant has appropriate medical coverage and that personal belongings are safeguarded during all activities.
LIABILITY WAIVER
In consideration of being permitted to participate in the Chess Club, I/We, the undersigned, for myself, my heirs, and personal representatives, hereby release and hold harmless West Fifth Bible Chapel, its leaders, organizers, and volunteers from all actions, claims, or demands for damages, injury, or loss arising out of participation in Chess Club activities, including those arising from negligence.
INDEMNIFICATION
I/We further agree to indemnify and save harmless West Fifth Bible Chapel and all its leaders, agents, and volunteers from any claims, demands, or expenses arising out of participation in Chess Club activities, including property damage or personal injury to others caused by the participant.
PURPOSE AND USE OF INFORMATION
West Fifth Bible Chapel is collecting and retaining this personal information solely for registration, communication, and emergency contact purposes related to the Chess Club. For questions about data use or to limit the information collected, please contact the Chapel directly.
I/We acknowledge that we have read and understood this waiver and release, and agree to its terms.
1. While every precaution is taken for the safety and good health of the Youth Conference participant, West Fifth Bible Chapel, its trustees, staff and volunteers, and the owner, management and employees of facilities used outside of the Chapel’s grounds are hereby released from any and all liability in the event of any illness, accident or misfortune that may occur to the participant. I understand the inherent risks involved in the activities during Youth Conference.
2. In case of medical emergency, I understand that every effort will be made to contact me, the parent/guardian of the participant. In the event that I cannot be reached, I hereby give permission to the physicians selected by West Fifth Bible Chapel Youth Conference directors to hospitalize and secure proper treatment, order injections, anesthesia, or surgery for my child, as named above. Provincial Health or equivalent Medical Insurance must cover each participant. The parent or guardian is responsible for any additional expenses that may result from such services.
3. To the best of my knowledge, my child is in good health and I will notify the Youth Conference directors if my child is unwell before or during Youth Conference.
4. I certify that my child is amenable and can accept reasonable discipline.
5. The Youth Conference directors reserve the right to dismiss a participant who, in his/her opinion, is a hazard to the safety and rights of others, or who appears to have rejected the controls of Youth Conference staff. I understand that my child will be given two warnings for misbehaviour. On the second misbehaviour, I will be called. If this behaviour occurs a third time, I will be called and expected to pick up my child.
6. I have read the above information and am in full agreement. The information I have given is true to the best of my knowledge.
